fix(db): 補建 ai_insights / action_plans 表
這兩張表從未存在於生產 DB，導致：
- Code Review 每次 _save_to_db() 靜默失敗
- get_history() 永遠返回空陣列
- /code-review/ 頁面歷史永遠空白
已直接在 production DB 執行 CREATE TABLE；
同步更新 docker/postgres/init/01-init.sql，
確保未來重建 DB 時自動建表。

- 建立 Gitea Actions CD pipeline (.gitea/workflows/cd.yaml)
- 部署模式: rsync Python 檔案至 188 → docker restart (volume mount)
- Dockerfile/requirements 變動時自動重建 Docker image

- 健康檢查: https://mo.wooo.work/health (最多 5 次重試)
